I certify that all of the above is true and I give the officers of Bethel Baptist Church permission to turn my information over to the appropriate agencies for contact tracing purposes as mandated by the NJ State Health Department. I agree to fully cooperate with all COVID-19 protocols required by Bethel Baptist Church, which are mandated by the State of New Jersey. Lastly, I will not hold Bethel Baptist Church or its officers liable should I contract COVID-19 resulting from attending service/event at the church.
